If you want to raise a concern or complaint about your care experience, you have the following options:

  • Speak with the care team member, supervisor/manager, or department director when you are concerned with any aspect of your care. They will make every reasonable effort to address and resolve any concerns to your satisfaction, while continuing to provide quality care. 
  • At any time, and/or if your concerns have not been resolved by your care team, you may contact Patient Relations by: 


Our Patient Relations Specialist serves as a neutral party dedicated to receiving, reviewing, and facilitating the resolution of patient complaints and grievances through appropriate follow-up and coordination.
