Department of Neuropsychology: Updated Background Information for Your Child's Evaluation

Before we schedule your child’s appointment, we need you to fill out a survey that will be helpful for the doctor who will see your child. The purple button above will take you to a secure website where you can give information about your child and provide ratings of his or her behavior. Thank you for taking time out of your busy day to give us this information. Getting this information ahead of time will help us be ready for your child’s appointment.

This is a shorter version of the questionnaire, and it takes most people about half an hour to finish. You do not have to finish the whole survey at one time. You can continue the survey at any time as long as you are on the same computer/tablet/phone and internet browser (e.g., Internet Explorer, Firefox, Google Chrome, etc.).

We use Qualtrics.com, a secure website, to collect information about our patients. Why can’t I go back while I’m taking the survey?
For data safety reasons, you are not allowed to go back to screens that you already passed. This prevents anyone from "backing up" and looking at your answers if you walk away from the computer.

What other steps have been taken to guarantee the safety and security of my answers?
We do everything we can to protect your information and reduce the risk of a breach of confidentiality.  You can read more about the electronic security measures on the Qualtrics website: www.qualtrics.com/security-statement. Your answers are encrypted to prevent anyone from seeing them when they are sent to us over the Internet. While it is HIGHLY unlikely that encrypted answers would be intercepted by someone on the Internet, answers would still be in a form that could not be read without a unique key that is held only by the database provider (i.e., Qualtrics).

Some of these questions might be good to ask my child’s teacher(s). Can the teacher fill out this survey?
This survey was created for parents to complete, but it is very helpful to have information from your child’s teacher. Once your appointment is scheduled, you will receive a letter for your child’s teacher. Please feel free to complete the permission form on that letter and give it to your child’s teacher(s), so that they can complete a separate teacher survey. The questions on the teacher survey ask about your child’s academic, social, and behavioral functioning at school.
