Outpatient Research Registry

The primary objective of this protocol is to gather information on Neuropsychology outpatients that can later be accessed for screening purposes (to determine eligibility for research studies conducted within the Neuropsychology Research Lab). Parents and caregivers of patients being evaluated in the Department of Neuropsychology outpatient clinic will be asked to complete the HIPAA form 3 as they arrive for their appointment and complete the sign-in process with the Care Center staff. We will be reviewing records to determine if the participant is eligible for any research studies. A recruitment database will be created which will include only the name and KKI number of individuals who agree to sign the HIPAA Form 3.

We will not collect any other information from the medical record for use in the recruitment database; rather, we will review the medical records of individuals who have signed Form 3s in order to determine eligibility for particular studies. The information in the recruitment database, as well as the information reviewed in the medical records will be used only for the purpose of assessing eligibility and to contact potential participants to determine whether they wish to participate in a study.
