When You Arrive

Providing your child with the best possible care is our first priority. To do so, we need accurate information about your child's medical history and needs. Please plan to be with your child as much as possible during the first several days, as our staff will have questions and information for you. When you register, you will be asked to complete various forms including consent for treatment, release of medical records, and protection of health information. For safety reasons, all patients are photographed and asked to wear identification bands upon admission.

A representative from the Finance Office will come to your child's room with admission paperwork that must be signed by the patient's legal guardian. Please bring all required medical and legal papers, along with the names and addresses of doctors and other providers who will need copies of your child's medical records.
